RTC reviews and investigates incidents on campus in order to establish preventative safety measures. RTC depends on the campus community to submit reports when incidents happen to help review and develop procedures in order to mitigate conditions and acts that may contribute to future incidents. Your report could help prevent a future accident or injury!
Report any work/instruction-related injury or illness to your supervisor as soon as possible. After notifying your supervisor, submit a report of the incident within 24 hours using the following form: RTC Near-Miss/Accident/Injury Online Report
Anyone can submit a near-miss, accident, or injury report at RTC.
Contact RTC Security immediately if the incident involves:
- An unsafe condition that will affect others
- In-patient hospitalization
